| LIFE CARE PLANNING
& TESTIMONY
Life Care Planning is a rehabilitation and long-term needs
assessment of the reasonable and necessary case-specific goods
and services required for an individual with a catastrophic
injury and/or chronic health care needs to maximize his or
her health and independence.
Life Care Plans are prepared by professionals with advanced
training in Life Care Planning, Catastrophic Case Management
and rehabilitation. Specific educational and professional
preparation in medical, nursing, therapeutic and rehabilitation
fields, coupled with nationally-recognized credentials in
Life Care Planning, Case Management and Rehabilitation Counseling
are indicators of proper preparation in determining the selection
of a Life Care Planner.

assessment, data analysis, and research which provides
an organized, concise plan for current and future needs
with associated costs, for individuals who have experienced
